When the defect is a little higher on the nose and away from the tip, the skin is more pliable and a single flap is rotated instead of two. This “transposition” flap transposes one triangle of skin into the defect. The first case demonstrates a combination of the single transposition flap (lower side-wall) and the bi-lobed flap (upper bridge).
